New Student enrollment is completed by appointment at the

Avon School Main Office: 505 Lincoln Avenue

Once all six (6) sections of documentation are prepared,

Section I: Proof of Residency/Domicile

Under state law, school officials may request proof that the adult enrolling the child lives within the boundaries of the school district. School districts typically accept a variety of documents for this purpose; see "Eligibility to Attend School" above.

1. Proof of Residency (choose one):

Guardian Residency Affidavit (own and live at a property in Avon)

Property Owner Affidavit (if you rent or stay at someone else's house)

Hardship Affidavit - Legal Guardian

Hardship Affidavit - Physical Guardian

Anticipated Residency Affidavit

2. Proof of Domicile:

Proof of Domicile Documention (evidence of personal attachment to a particular location)

3. Enrollment Type (identify one that best matches the student's circumstances - guidance ):

Section A - Domicile

Section B - Affidavit Student

Section C - Temporary Resident

Section D - Special Circumstances

Court order, State agency agreement, or other documentation of court or agency placement or directive

All students are required to have had a physical examination within the year prior to the start of school. Additionally, the Avon School District adheres to the immunization requirements of the state of New Jersey. Utilize the forms and information below to complete the Health Office requirements for enrollment:
