• Sport Physical Examinations
     
    Students who wish to participate in school team sports must have a yearly Physical Examination aka: "Physical" on file in the health office. There is a specific Physical Examination Form, approved by the state, that is required (see the Athletic Forms link). There are two ways families can obtain Physical Examination for their child:
     
    Private Physical Examination
     
    Parent/guardian may decide have a private Physical Examination performed for their child.
    1. Complete the Preparticipation Physical Evaluation History form (and the Athletes with Special Needs Supplemental History Form if indicated)
    2. Submit the history form(s) to your child's healthcare provider prior to the examination. 
    3. After performing the examination, the healthcare provider needs to completed and signed both the physical examination page and the clearance page. 
    4. Once both forms have been completed, submitted them along with the history form(s) to the Avon School Health Office.
    5. The private physical examination forms need to be cleared by the school physician prior to participation. 
     ** As of Spring 2015 there is a new state requirement for healthcare providers performing sport physical examinations.
     
    In-School Physical Examination
     
    Parents/guardians may choose to have the Physical Examination performed by the school physician. In order to do so, complete, sign, and submit the following::
    •       Pre-participation Physical Evaluation History (and Athlete with Special Needs Supplemental History form, if indicated)
    •       Examination Consent Form - allows school physician to perform the examination
    No in-school physical examination will be performed without these forms listed above.
     
    Due Dates for form submission In-School Examination date will be posted on the Health Office Calendar.
     
    Updates
     
    Prior to the start of every sport season (Fall, Winter, and Spring), parents/guardians are required to complete, sign, and submit a Health History Update Form. This form must be on file in the health office prior to the start of the season. No new physical examination is required for the update.
