Section I: Proof of Residency/Domicile
Under state law, school officials may request proof that the adult enrolling the child lives within the boundaries of the school district. School districts typically accept a variety of documents for this purpose; see "Eligibility to Attend School" above.
1. Proof of Residency (choose one):
- Guardian Residency Affidavit (own and live at a property in Avon)
- Property Owner Affidavit (if you rent or stay at someone else's house)
- Hardship Affidavit - Legal Guardian
- Hardship Affidavit - Physical Guardian
- Anticipated Residency Affidavit
2. Proof of Domicile:
- Proof of Domicile Documention (evidence of personal attachment to a particular location)
3. Enrollment Type (identify one that best matches the student's circumstances - guidance):
Court order, State agency agreement, or other documentation of court or agency placement or directive
