Medication
For the safety of all students, medication should never be sent to school with a student. All medications—both prescription and over-the-counter—must be accompanied by a written order from a physician to be administered during school hours.
Additionally, all medication must be brought to the school nurse by an adult, and must be in its original container.
Asthma Action Plan - If your child requires Asthma medication in school, please have the doctor complete the Asthma Action Plan. BOTH the parent and the physician must sign this form.
Food Allergies Action Plan - If your child requires an EPI-PEN in school, please have the doctor complete the Food Allergies Action Plan. BOTH the parent and the physician must sign this form.
Medication Form - If your child requires any other medication in school (prescription or "over-the-counter" ie,, tylenol, advil, cough drops), please have the doctor complete the medication form.
